  • Complete and submit the Contact Us form to create a service ticket for the 4Next support team.
  1. All fields are required. Your name and contact information is pulled from your 4Next user profile.
  1. Click to select an option from the Type of Problem drop-down list.
  2. Type a description of the problem you're having.
  3. Click submit. Once the service ticket is sent:
  • A message displays to indicate that your request was sent. 
  • If you need to send another ticket, click Contact Us. Otherwise, use the main navigation to continue.

  • Monday through Friday 8:30am – 5:00pm
  • Weekend & holiday on call for patient-care critical issues only 8:30am – 5:00pm

Response time for patient-care critical issues is 10 minutes. For all other issues, 4Next support will respond to your ticket within 1 business day.
